Sakit Gigi in Kota Kinabalu — what it means and what to do
A toothache is a symptom, not a diagnosis. Something specific is causing it — decay reaching the nerve, a cracked tooth, an infected root, a wisdom tooth pushing through, or gum inflammation. Painkillers quiet it; only treating the cause ends it. This page explains what your pain is likely telling you, when it is an emergency, and what we can do about it at FTS Chin Dental in Likas.
When a toothache is a medical emergency
Most toothache is urgent but not dangerous. A small number of cases are dangerous, and they are the ones where waiting for a dental appointment is the wrong move. If any of the following apply, go to a hospital emergency department now. In Malaysia, the emergency number is 999.
- Swelling spreading across your face or down into your neck — especially if it is growing over hours rather than days.
- Difficulty swallowing, or any difficulty breathing.
- An eye swelling closed on the same side as the painful tooth.
- Fever together with facial swelling.
- You cannot open your mouth properly — jaw stiffness alongside a painful tooth.
- Bleeding you cannot stop after a tooth has been taken out.
- A permanent tooth knocked completely out. This one is time-critical: hold it by the crown, never the root, keep it in milk or the patient's own saliva, and get to a dentist or hospital as fast as you can.
These signs can mean a dental infection has started spreading beyond the tooth. That is treated as a medical emergency rather than a dental one, and it needs a hospital rather than a clinic appointment. We would much rather you went there than waited politely for us to open.
What is causing my toothache?
The character of the pain is a genuine clue. It does not replace an examination — two people with identical symptoms can have different problems, and an X-ray often settles it — but it does tell you roughly how urgent this is and what treatment is likely.

What can I do at home until I get to a dentist?
These measures make the wait more bearable. None of them treats the cause, and none of them is a reason to delay being seen — a tooth that has started hurting rarely settles permanently on its own.
- Rinse with warm salty water — about half a teaspoon of salt in a cup of warm water, several times a day.
- Take a painkiller you already know you tolerate, at the dose on the packet. If you are unsure what is safe for you, ask a pharmacist.
- Sleep with your head raised on an extra pillow. Lying flat increases the pressure on an inflamed nerve, which is a large part of why toothache is worse at night.
- A cold compress on the outside of the cheek for swelling — twenty minutes on, twenty off.
- Keep the area clean but gentle. Keep brushing, softly, even where it is sore.
- Avoid what sets it off — very cold drinks, very hot drinks, sugar, and chewing on that side.
Two things that make it worse
Do not hold aspirin or a painkiller tablet against the gum. It is a common piece of advice passed around and it is wrong — painkillers work through the bloodstream, not by contact, and a tablet held against the gum causes a chemical burn on top of the toothache you already have. Swallow it.
Do not put heat on a swollen face. Warmth feels soothing on an ache, but if there is swelling, heat encourages an infection to spread rather than settle. Cold on the outside, warm salt water on the inside.
What if it starts at night, or on a Sunday?
We are a single clinic in Likas with ordinary opening hours, and we would rather say so plainly than imply a 24-hour service we do not run.
- Monday to Wednesday, Friday and Saturday: 9am to 5pm.
- Thursday: 9am to 1pm — half day.
- Sunday: closed.
If the pain starts outside those hours, use the measures above, and message us on WhatsApp anyway — the message will be waiting when we open and we will prioritise someone who has been in pain overnight. If any of the red flags at the top of this page apply, do not wait for us at all; go to a hospital emergency department.
What happens when I come in with a painful tooth?
The first job is finding out what is actually wrong, because the same pain can have several causes and the treatments are different.
- We ask what the pain is like, what sets it off, how long it lasts and whether it wakes you — the answers narrow it down considerably.
- We examine the tooth and the gum around it, and test how it responds.
- We usually take an X-ray. Decay under a filling, a crack, or infection at the root tip are frequently invisible without one.
- We explain what we have found and what the options are, including doing nothing for now and what that risks.
- You are told the price before treatment begins, not after.
Where we can save a natural tooth, that is what we aim for — a tooth of your own is better than any replacement. Where it genuinely cannot be saved, we will say so directly rather than putting you through treatment that will not hold.

Tooth Pain
Toothache genuinely does get worse at night, for two physical reasons. Lying flat raises blood pressure in the head, which increases pressure inside an already inflamed tooth. And with nothing else competing for your attention, pain that was manageable at 3pm becomes the only thing you can think about at 3am. Pain that wakes you usually means the nerve inside the tooth is involved.

The one that matters most is swelling that spreads beyond the gum into the face or neck — that is a medical emergency, not a dental appointment.

Sensitivity happens when dentine — the layer under the enamel, full of microscopic tubes leading to the nerve — becomes exposed. Cold, sweet or acidic things then reach the nerve far more easily. The useful test is duration: a sharp twinge that stops within seconds is ordinary sensitivity, while an ache that lingers a minute or more after the cold has gone suggests something in that specific tooth.

The useful distinction is between a problem that needs a hospital tonight — spreading facial swelling, difficulty swallowing or breathing, a knocked-out permanent tooth — and one that is painful but can be managed until morning.
